I agree to the terms below for me and my child(ren)

I /We acknowledge and understand that participating in programs, activities, and events held by OH What a Space, LLC involves inherent risks, including but not limited to accidents, injury, illness, and death and risks either not known to me or not easily foreseeable at this time. I/We voluntarily assume all risks associated with participation, including but not limited to, falls, contact with other participants, the effects of the weather and all other such risks being known and appreciated by me.

 

II/We recognize my/our responsibility to communicate any physical or psychological conditions that may affect my/our ability to safely participate in these activities. I/We confirm that I/we are physically and mentally capable of engaging in the activities I/we choose to participate in.

 

Having read and understood this waiver, and in consideration of my/our participation, I/We hereby release, discharge, and hold harmless OH What a Space, LLC, its representatives, and successors from any liability, claims, or demands arising from my/our participation in these activities.

 

As parent/guardian, I consent to have my child receive First Aid by faculty, staff and if necessary, be transported to receive emergency care. I will be responsible for all charges not covered by insurance. I consent for the emergency contact persons listed above to ACT ON MY BEHALF until I am available. I agree to review and update this information whenever a change occurs or every 12 months.

 

I understand that it is my responsibility to update this form in the event that I no longer wish to authorize one or more of the above uses. I agree that this form will remain in effect during the

term of my child’s enrollment.

 

After having read this waiver and knowing these facts, and in consideration of acceptance of my participation and by my signature I/We indicate that I/We have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.

 

 

Facility Rules, Terms and Conditions

PARENTAL SUPERVISION - Parental supervision is REQUIRED at all times for all “Stay and Play” and programs labeled Family Events. OH What A Space staff is NOT responsible for supervising children. Parent/Guardian should be present at all times. No rough play or fighting.

 

POTTY TRAINED-All children who attend any of our drop off programming need to be completely potty trained and independent at using the restroom.

 

FOOD POLICY - No outside food is allowed in our public areas unless part of a private event.

 

CANCELLATIONS/REFUNDS - All event registrations are non-refundable if not canceled by purchaser at least one week prior to the event or if the facility is unexpectedly closed/program canceled. Refunds will not be given when any session is missed during a multisession program.

 

SICK POLICY - Please do not bring sick children or adults to the facility. Any visibly ill visitors may be asked to leave to ensure that we have a safe environment for all. In the event that a visibly ill visitor is asked to leave, a refund will not be issued.

 

SOCKS - Some programming may require no shoes and for those programs, socks are required for all.

 

OUR RIGHTS – OH What A Space, LLC  reserves the right to ask children/parents to leave if visitors do not follow the rules above or visitors who are being harmful to others.

 

OH What A Space, LLC  also reserves the right to ask children/parents to leave in the event of bullying or harassing conduct including, but not limited to, racial and sexual harassment. In the event that a visitor is asked to leave for violation of this term, no refunds will be issued.

 

Photo and Video Release

We're excited to have your child(ren) join us at OH What A Space, LLC. To help us share the fun and progress your child(ren) makes, we'd like your permission to use photos and videos taken during their time here.

I/We give permission for OH What a Space, LLC, its employees, contractors and/or assigns to photograph my child, for the following purposes: Photos and videos for our website, social media, education and marketing purposes, payment free. We will not share any personal information about your child including names.

 

Child Pick Up

I give the named people to pick up and transport my child/(ren) for OH What a Space, LLC.  I hereby inform OH What A Space LLC, its employees, agents, contractors and/or assigns that the

people listed below are authorized to pick up the above named child/ren at any time.  Accordingly, OH What A Space, LLC, its employees, agents, contractors and/or assigns is hereby instructed to

release my child/ren into the care of the named people whenever they come to the Facility.  AUTHORIZED PICK-UP PERSON (must be at least 18 years old unless special written arrangements are made ahead of time).

 

Any person that picks up your child/ren may be asked to provide a photo ID to the staff if they are not familiar with the person on the above list.
